1910.23(a)(9): Every floor hole into which persons can accidentally walk was not protected by a cover that leaves no openings more than 1 inch wide.     On or about January 13, 2016, at a water tower renovation site, employees were allowed to work in the tank of a water tower that had a cover over the floor opening with openings more than one inch wide, exposing employees to the hazard of falls.
