29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent.   On or about 2/8/2016, at 3661 Eisenhower Parkway, Suite 50 in Macon, GA:  The employer did not record the following workplace injuries and illnesses on the OSHA 300 log for calendar year 2015. On or about 12/19/2005, Server - An employee left thumb was lacerated while cutting steak meat with a knife toward himself, causing the knife to cut the employees left thumb.  The injury required treatment by a physician on 12/23/2015.  The injured employee was placed on restricted duty by the physician and not recorded on the OSHA 300 log as a Work Related Injury or Illnesses for calendar year 2015.
