The work platform was moved horizontally while occupied and was not securely attached to the forklift.

Employees were exposed to the hazard of falling 14-feet to the ground below. 29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Cross braces were used as a means of access. (a) Tubular Welded Scaffold near residential construction site, during masonry operations, an employee, who was working on a tubular welded scaffold and the cross braces on tubular welded frame scaffolds were used as a means of access or egress. The employee was exposed to a fall hazard of approximately 16-feet above the lower level. Gustavo Quintanilla Ramos dba Quintanilla Masonry was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(e)(1), which was contained in OSHA inspection number 1045483, citation number 01, item number 003, and was affirmed as a final order on November 23, 2015, with a final abatement date of November 23, 2015 with respect to a workplace located at 604 Dividend Plano, TX 75074.
