29 CFR 1926.451(d)(3)(ix): Tiebacks were not secured to a structurally sound anchorage on the building or structure  a) On or about 02/03/2016, at the above addressed work site, a tieback for a suspension scaffold was attached to the side of an eye bolt installed vertically on a wall and not secured to a structurally sound anchorage on the building exposing employees to a fall hazard of approximately 50-feet.   b) On or about 02/03/2016, at the above addressed work site, a tieback for a suspension scaffold was attached to an eye bolt and anchor which was partially pulled out of the concrete and not secured to a structurally sound anchorage on the building exposing employees to a fall hazard of approximately 50-feet.

29 CFR 1926.451(d)(3)(x): Tiebacks were not installed perpendicular to the face of the building or structure, nor were opposing angle tiebacks installed.  a) On or about 02/03/2016, at the above addressed work site, tiebacks for a suspension scaffold on the north side of the building were installed an angle and not installed perpendicular to the face of the building exposing employees to a fall hazard of approximately 50-feet.   b) On or about 02/03/2016, at the above addressed work site, a tieback for an outrigger scaffold on the south side of the building was  installed an angle and not installed perpendicular to the face of the building exposing employees to a fall hazard of approximately 50-feet.

29 CFR 1926.451(d)(12)(i): There was not a minimum of 3 wire rope clips installed, with the clips a minimum of 6 rope diameters apart:  a) On or about 02/03/2016, at the above addressed work site, tiebacks for parapet clamp scaffolds on the north side of the building did not have a minimum of three wire rope clips exposing employees to a fall hazard. of approximately 50 feet.   b) On or about 02/03/2016, at the above addressed work site, tiebacks for parapet clamp scaffolds on the pool deck did not have a minimum of three wire rope clips exposing employees to a fall hazard of approximately 50-feet.
