29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:    (a)  Wadsworth & Associates, Inc. dba Wadsworth Solutions Northwest on or about February 08, 2016, at the address of 420 Madison Ave., Toledo, OH:  The employer failed to ensure an employee tasked with performing a site inspection/evaluation of HVAC controls at the aforementioned address was adequately protected from a fall by ensuring the use of appropriate fall protection equipment. The employee performing these task in a confined space (Air Handler #1) in which maintained a unprotected edge to a lower level, was exposed to an approximant 7.5 ft. fall to concrete surface below.

29 CFR 1910.146(c)(3): When the employer decided its employees would not enter permit spaces, the employer did not take effective measures to prevent its employees from entering the permit spaces:    a)  Wadsworth & Associates, Inc. dba Wadsworth Solutions Northwest on or about February 08, 2016, at the address of 420 Madison Ave., Toledo, OH: The employer failed to take effective measures to ensure that employees who are not expected to enter a permit entry confined space avoid theses spaces by effectively evaluating the workplace and informing affected employees of the location and hazards contained within, prior to work commencing. Due to this condition, an employee performing an inspection/site survey of ventilation sensors in a permit confined space was exposed to unexpected energization of forced air systems and fall hazards.

The employee operating in the space without the benefit of energy isolation of the forced air blowers exposed the employee to potential entrapment and fall hazards in the event of unexpected blower startup.

The employee was operating in the space without the benefit of energy isolation of the forced air blowers, exposing the employee to potential entrapment and fall hazards in the event of unexpected blower startup.
