29 CFR 1910.178(a)(4): Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:                                                           a) Main press room, 730 4th Avenue, Coraopolis, PA 15108:  On or about February 10, 2016, the employer did not obtain prior written approval from the forklift manufacturer for the use of a wooden platform used to elevate employees on a Hyster brand forklift (model number S50E, serial number D002D10414B) in the main press room.  Employees lifted on the forklift platform were exposed to a fall hazard of up to approximately 20 feet to the concrete floor below.

29 CFR 1910.303(g)(1)(ii):  Working space about electric equipment rated 600 volts, nominal, or less was used for storage:   a) Back of shop near book binder, 730 4th Avenue, Coraopolis, PA 15108:  Observed on or about February 10, 2016, access to electrical panels located near the book binder in the back of the shop was restricted by the storage of several hoppers.  Each hopper contained paper products that were awaiting to be recycled.  The electrical panels were used to supply power to various type of equipment throughout the facility.

29 CFR 1910.305(a)(2)(ix):  All lamps for general illumination were not protected from accidental contact or breakage by a suitable fixture or lampholder with a guard:  a) Main press room, 730 4th Avenue, Coraopolis, PA 15108:  Observed on or about February 10, 2016, the light bulbs on a light fixture located near a storage rack used to store a forklift basket were not protected from accidental contact or breakage by a suitable fixture or lampholder with a guard.  The forklift operates within a few inches of the light fixture while moving a forklift basket.  Employees were exposed to the hazards of electric shock and cuts and scrapes from broken glass in the event that the light bulbs would break.
