29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:     On or about February 10, 2016, and at times prior thereto, employees were allowed to operate machines that were found not having guards provided, including but not limited to the following instances:     1) In the tool & die area, an engine lathe machine was being operated without a chuck guard provided to protect operator from exposure to the chuck while in operation.     2) At the trash compactor area, a trash compactor being was operated without having an interlock device or a dead-man switch provided to prevent employees from entering into the trash compactor while in operation.    Employees were exposed to amputation and crushing hazards.

29 CFR 1910.212(a)(2): Guard(s) on machine(s) were not affixed to the machine or secured elsewhere when attachment to the machine was not possible:     On or about February 9, 2016, and at times prior thereto, employees were allowed to operate machines that were found having the guards were not secured, including but not limited to the following instances:   1) In the tubing area, a T-drill machine was observed having the guard was modified and not secured and could  be opened easily without the use of any special tool.     2) In the expander area, expender machine #1 was observed having the guard not secured and could  be opened easily without the use of any special tool.   Employees were exposed to amputation hazards.

29 CFR 1910.217(c)(1)(i): The employer did not provide and ensure the usage of point of operation guards or properly applied point of operation devices on every operation performed on mechanical power press(es):    On or about February 9, 2016, and at times prior thereto, in the fin press area employees were operating mechanical power presses that had guards provided but did not properly protect employees from exposing to "point of operation".  Employees were exposed to an amputation hazards.
