29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident,  the in-patient hospitalization, amputation, or loss of an eye was not reported OSHA:    29 CFR 1904.39(a)(3): You must report the fatality, in-patient hospitalization, amputation, or loss of an eye using one of the following methods: (i) By telephone or in person to the OSHA Area Office that is nearest to the site of the incident. (ii) By telephone to the OSHA toll-free central telephone number, 1-800-321-OSHA (1-800-321-6742). (iii) By electronic submission using the reporting application located on OSHA's public website at www.osha.gov.    a) Jobsite, 200 Neville Road, McKees Rocks, PA:  On or about February 4, 2016, an employee was hospitalized due to a work place injury.  He was released on February 5, 2016 and the employer did not call to report it to OSHA until February 8, 2016.

29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment was not physically located and operated in such a manner as to isolate the machine or equipment from the energy source:    a) Jobsite, 200 Neville Road, McKees Rocks, PA:  On or about February 4, 2016, an employee was injured while attempting to unclog the tip on a Briggs & Stratton 4,000 psi power washer, model number 020507, because all of the energy sources where not isolated and depressurized.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control:    a) Jobsite, 200 Neville Road, McKees Rocks, PA:  On or about February 4, 2016, an employee was not trained on the appropriate method, according to the manufacturer's instructions, for cleaning and/or changing the spray tip and was injured while attempting to remove the tip on a Briggs & Stratton 4,000 psi power washer, model number 020507, because the trigger lock was not engaged and the water that was stored in the wand was not depressurized and shot the tip off and water which lacerated the employees left middle finger.
