29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: On or about November 17, 2015 at the 1157 Boon Lasseign Road, Saint Martinville, LA job site; the employer (general contractor) allowed the employee of a subcontractor (truck driver) to transport ground mats; the employer failed to provide to the employee site safety orientation and daily Job Safety Analysis (JSA) toolbox training.
