29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service:    (a) On February 11, 2016, at a jobsite at 9503 N. Congress Avenue, Kansas City, MO, an employee was exposed to serious hazards of falls of approximately 15-20 feet from an aerial lift. The personal fall arrest system had not been inspected prior to use for wear, damage, and other deterioration, and defective components were not removed from service; the lanyard was visibly worn and frayed.

29 CFR 1926.503(a)(2)(ii): The employer did not assure that each employee, as necessary, had been trained by a competent person in the correct procedures for erecting, maintaining, disassembling and inspecting the fall protection systems to be used:    (a) On February 11, 2016, at a jobsite at 9503 N. Congress Avenue, Kansas City, MO,  an employee was exposed to serious hazards of falls of approximately 15-20 feet from an aerial lift. The employee was not trained by a competent person in the correct procedures for erecting, maintaining, disassembling and inspecting the fall protection systems to be used, in that the harness was incorrectly fitted, and the lanyard was visibly worn and frayed, and no inspection of the system had taken place prior to use.
