29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:  (a)On February 11, 2016, at a jobsite at 2709 Grove Street, Kansas City, MO, employees were exposed to an electric shock hazard in that the extension cord plugged into the portable gas generator and powering the DeWalt circular saw did not have its grounding pin plugged into the receptacle, interrupting the path to ground.  NOTE: Abatement certification is required for this item - use CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  (a)On February 11, 2016, at a jobsite at 2709 Grove Street, Kansas City, MO, employees were exposed to a fall hazard of approximately 11 feet, 11 inches in that they were conducting roofing work on a steep roof without any form of fall protection.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REAQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a)On February 11, 2016, at a jobsite at 2709 Grove Street, Kansas City, MO, employees were exposed to a fall hazard of approximately 11 feet, 11 inches in that the side rails of a ladder used to access the roof did not extend at least 3 feet above the edge of the roof.
