29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems or personal fall arrest system:       a) Jobsite - Employees engaged in the repair of a saw-tooth type roof were exposed to fall hazards of up to twenty-seven (27) feet due to a lack of personal fall arrest systems, guardrail systems, or safety net systems. 29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a) Jobsite - The employer did not ensure that employees using a ladder to access the upper landing surface had at least 3 feet of the side rails of the ladder extended above the landing surface to aid in the mounting and dismounting of the ladder.  The height from grade to the roof was approximately twenty-seven (27) feet and the ladder used had a maximum working length of twenty-nine (29) feet. Noted on February 4, 2016.
