29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    On or about 02/12/2016, at 9998 Crossing Way, Denham Springs, LA, employees were conducting roofing work on the balcony of a commercial theater building at a height of approximately 16 feet without a means of fall protection.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:    On or about 02/12/2016, at 9998 Crossing Way, Denham Springs, LA, employees were conducting roofing work on the balcony of a commercial theater building at a height of approximately 16 feet without a secured ladder to access the balcony.
