(a) Located on the vessel Hanjin Isabel berthed at 14003 N Rivergate Blvd in Portland: On or about and at times prior to February 11, 2016 the inspection revealed that the spreader bar used to lower bucket excavators into the hold did not have a legible safe working load marked on it.

(a) Located on the vessel Hanjin Isabel berthed at 14003 N Rivergate Blvd in Portland: On or about and at times prior to February 11, 2016 the inspection revealed that the spreader bar used to lower bucket excavators into the hold had not been proof load tested as required by the standard.

29 CFR 1918.81(b): Other positive means were not taken to prevent cargo handling bridles from being accidentally disengaged from the cargo hook.   (a) Located on the vessel Hanjin Isabel berthed at 14003 N Rivergate Blvd in Portland: On or about and at times prior to February 11, 2016 the inspection revealed that employees were potentially exposed to serious injury because the crane hook connected to the clamshell excavator attachment was not secured by a positive means to prevent accidental disengagement.
