29 CFR 1910.132(a) Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used, and/or maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact: a) CNC machine department: The employer did not ensure that employees used protective equipment provided, such as, Tyvec sleeves or Corium Hand and Skin Barrier cream, when removing product from CNC machines that used SSW 3-2 cutting oil. Employees have developed irritation and rashes on theirs arms, on or about 2/17/16. b) CNC machine department: The employer did not ensure that employees used protective equipment provided, such as, Tyvec sleeves, when removing product from CNC machines that used TRIM OGR 263-X, a corrosive chemical. Employee have developed irritation, rashes and burning sensations on their arms, on or about 2/17/16. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

Machine operators retrieve the completed burs for inspection without disabling all of the energy to the moving parts of the machine, such as, but not limited to, the diamond wheel and the load arm, and bypassing guards and interlocks, on or about 2/17/16. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
