29 CFR 1910.95(c)(1): The employer had not administered a continuing, effective hearing conservation program, as described in paragraphs (c) through (o) of this section, whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level (TWA) of 85 decibels measured on the A scale (slow response) or, equivalently, a dose of fifty percent: On or about March 22, 2016, the employer had not established a hearing conservation program for at least the grinder operator and blaster operator, plant 3, who were exposed to noise at 106% and 143.9%, respectively, equivalent to a noise level of 93.97 dBA and 91.29 dBA, respectively. The noise levels were above the action noise level of 85 dBA. Exposure calculation included zero increments for the time not sampled.

The flexible cord was run along the trusses and connected to an unrated power strip.
