29 CFR 1910.119(j)(5): The employer did not correct deficiencies in equipment that were outside acceptable limits (defined by the process safety information in paragraph (d) of this section) before use:    Claremont Terminal - On or about June 8, 2016, a support for the vapor and liquid piping used to deliver LPG from the rail cars to four of the 90,000 gallon LPG storage vessels, was observed to be positioned at an angle, which deviated from the intended vertical position, causing it to no longer support the piping.
