29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  a)  On or about February 16, 2016 at 3017 Tri-Park Court in Appleton, employer failed to provide fall protection safety training certification for employees working at heights greater than 6 ft.  Employees did not recognize fall arrest hazards from using a personal fall arrest system with the anchor point installed below the walking/working surface, with the body harness D-ring positioned below the shoulder blades, and with the lifeline caught on and/or around materials/machinery.
