29 CFR 1910.147(c)(7)(i)(A): Each authorized employee did not receive training in the recognition or applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control: A. On or about February 22, 2016, Production Shop: Employees who were authorized to lock out equipment for repair or maintenance had not been provided with specific training. Employees perform servicing and maintenance activities on equipment including, but not limited to, Hypertherm plasma cutter, Lockformer water jet, Iowa Precision Cornermatic corner former, and Iowa Precision Coil Line, which had multiple energy sources such as pneumatic, hydraulic and electric.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area. A. On or about February 22, 2016, Production Shop: Employees working in the shop had not been included in the hazard communication training that was provided to field employees. Employees working in the shop were exposed to chemicals including, but not limited to ethylene glycol, ammonia, methanol, xylene, acetone and propane.
