On or about 02/18/2016 at the Kingswood Place Construction Project, employees were working from an upper floor deck with no fall protection system in use.  Employees were also working at and near unguarded window openings.

On or about 02/18/2016 at the Kingswood Place Construction Project, an employee was carrying a 2x12 piece of lumber on his shoulder and was using his right hand to leap from rung to rung while climbing up the ladder.
