29 CFR 1926.102(a)(1):   Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:   (a) On or about February 16, 2016 at worksite 9205 N Dawn Ave Kansas City Missouri.-employees were exposed to serious eye injury while using a Hibachi 5008AC2 model pneumatic nail gun without eye protection.    Abatement certification for this item is not required.

29 CFR 1926.501(b)(1):       Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.      (a) On or about February 18, 2016 at 9205 N Dawn, Kansas City Missouri 64116. Employees was exposed to a fall hazard while working at the edge of a roof surface approximately 11 feet without Fall Protection Equipment or Fall Protection Systems  in use.       Abatement certification for this violation is not required.  .
