29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets. (a) Construction Site - On or about February 12, 2016 two employees were exposed head injuries while other employees were working on scaffolds above them handling bricks. Employees on-site were not wearing hardhats.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports as follows: (a) Northside of Building - On or about February 12, 2016 an employee was exposed to a fall hazard due to the platform not being fully decked or planked. There was a 19 inch gap where a deck was not installed.

29 CFR 1926.454(b): The employer shall have each employee who is involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work in question. The training shall include the following topics, as applicable: (a) Construction Site - On or about February 12, 2016 three employees was exposed to fall hazards while working on scaffolds. Employees were not provided training on how to erect, disassemble, move, maintain, repair and inspect scaffolds.
