Employees were exposed to unexpected release of energy hazards.

29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing or blocking of machines or equipment from energy sources: On or about February 16, 2016, and at times prior thereto, at the sorter conveyor in the Distribution II building the employer did not provide group lock-out/tag-out hasps or similar equipment for multi-employee lock-out to employees performing maintenance of the sorter conveyor. The employees were exposed to hazards caused by unexpected start-up of machines or equipment.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about February 16, 2016, and at times prior thereto, in the Distribution II building employees were allowed to ride on the sorter conveyor where the openings (windows) were not guarded to prevent contact with moving parts. Employees were exposed to injury caused by moving parts.
