29 CFR 1910.23(a)(9): Every floor hole into which persons can accidentally walk (on account of fixed machinery, equipment, or walls) was not protected by a cover that left an opening more than 1 inch wide. The cover shall be securely held in place to prevent tools or materials from falling through:    (a)  On or about February 19, 2016, and at times prior to that date, employees were exposed to potential fall hazards where the walking/working surface on the distribution floor was missing a cover exposing a floor hole measuring 12" by 12".    (b)  On or about February 19, 2016, and times prior to that date, employees were exposed to potential fall hazards where the distribution floor hole covers were not secured in place.
