29 CFR 1910.67(c)(2)(ii): An employee(s) operating an aerial lift(s) was not properly trained:  On or about February 23, 2016, at the above addressed site, an employee performing banner exchanging duties from  the basket of an aerial lift exposed to a fall hazard was not provided with a training program to enable him to recognize the hazards associated with falls.   PLEASE NOTE:  CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.

29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift:   On or about February 23, 2016, at the above addressed site, an employee performing banner exchanging duties was exposed to a fall hazard of approximately 15 feet 6 inches when working from the basket of an aerial lift while not wearing a harness with a lanyard attached to the boom or basket of that lift.
