29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.      (a) Front of building facing street:  An aluminum step-ladder was used to access the roof area where employees were installing new roof shingles.  The ladder did not extend three (3) feet above the landing and was not secured exposing employees to a fall of approximately twelve (12) feet to the debris covered pavement below.    (b) Rear of building: An aluminum extension-ladder was used to access the roof area where employees were installing new roof shingles.  The ladder was leaning against carpenter bracket scaffolding, did not extend three (3) feet above the landing, and was not secured exposing employees to a fall of approximately thirty (30) feet to the debris covered frozen ground below.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:    (a) Rear of building:  An aluminum step-ladder was laid on the roof of the building held in place by a piece of wood and used by employees to access the upper roof area.  The ladder was not intended for this use and exposed employees to a fall of approximately thirty (30) feet to the ground below.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     Jobsite: An employee was on the peak of a steep-pitched roof laying out roofing shingles without the use of fall protection exposing him to a fall of approximately thirty (30) feet to the pavement below.    Guaman General Services, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 315141119, citation number 1, item number 1, and was affirmed as a final order on October 28, 2011, with respect to a workplace located at 5 Romsey Street, Dorchester, Massachusetts.
