(a) Front of building facing street:  An aluminum step-ladder was used to access the roof area where employees were installing new roof shingles.  The ladder did not extend three (3) feet above the landing and was not secured exposing employees to a fall of approximately twelve (12) feet to the debris covered pavement below.    (b) Rear of building: An aluminum extension-ladder was used to access the roof area where employees were installing new roof shingles.  The ladder was leaning against carpenter bracket scaffolding, did not extend three (3) feet above the landing, and was not secured exposing employees to a fall of approximately thirty (30) feet to the debris covered frozen ground below.
