29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  Location: 252 Monroe Turnpike, Monroe, CT 06468  Each employee required to work from an aerial lift did not receive scaffold training and information by a person qualified in scaffolds and aerial lifts in order to understand the procedures to minimized and/or eliminate hazards associated with scaffold and aerial lifts.
