29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.    a. On or about February 24, 2016, employees were required to cut aluminum stock with radial arm saw(s) with exposed blades. The bottom portion of saw blade was not properly guarded. Thereby exposing employees to cuts, lacerations, and amputation hazards.

29 CFR 1910.303(b)(1)(iv): Electric equipment shall be free from recognized hazards that are likely to cause death or serious physical harm to employees. Safety of equipment shall be determined using the following considerations: Electrical insulation;    a. Production Floor- On or about February 24, 2016, employees were required to cut aluminum stock with a miter saw. The outer sheathing on the miter saw's power cord was frayed and it's insulation was worn. The power cord was plugged into an energized electrical outlet. Employees were thereby exposed to shock and electrocution hazards.

The OSHA 300 form is called the Log of Work-Related Injuries and Illnesses, the 300-A is the Summary of Work-Related Injuries and Illnesses, and the OSHA 301 form is called the Injury and Illness Incident Report.    a. Throughout facility- On or about February 24, 2016, the employer failed to keep OSHA 300 logs, summaries, and injury and illness reports for 2015 and 2016 as required by the standard.

Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA.    a. On or about February 9, 2016, the employer did not contact OSHA within 24 hours when an employee at the facility sustained a work-related injury and was admitted to the hospital as an in-patient hospitalization.
