29 CFR 1926.303(b)(1): Grinding machines were not equipped with safety guards in conformance with the requirements of ANSI B7.1-1970, Safety Code for the Use, Care, and Protection of abrasive wheels: a) 100 Norfolk St., NY, NY - 3rd floor: On or about 02/23/2016, two MDI Construction employees were operating a Chicago Electric angle grinder with abrasive blade to cut and shape structural steel members prior to welding. The blade of the grinder was not guarded in any manner, exposing the employee to laceration hazards.

Wooden plugs driven into holes in masonry, concrete, plaster, or similar materials shall not be used. b) 100 Norfolk St., NY, NY- 4th floor: On or about 02/23/2016, an MDI Construction employee using a PowCon portable electric welder was exposed to potential shock hazards due to the electrical circuit box not being properly mounted and firmly secured.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: a) 100 Norfolk St., NY, NY- 3rd floor: On or about 02/23/2016, two MDI Construction employees using a Hougen magnetic drill were exposed to potential shock hazards due to the lack strain relief on flexible extension cord used as a temporary source of electrical power . b) 100 Norfolk St., NY, NY- 4th floor: On or about 02/23/2016, an MDI Construction employee using a PowCon portable electric welder was exposed to potential shock hazards due to the lack strain relief at the plug end of the units power cord.
