29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, warning line system and personal fall arrest system, or warning line system and safety monitoring system; or, on roofs 50 feet (15.25 m) or less in width, the use of a safety monitoring system alone: 3505 W 13th St., Grand Island, NE 68803 (Job-site): The employer failed to ensure that that the safety monitor was only performing the task monitoring of roofing employees. On 02/24/16, employees were working off of a roof that was approximately 15 feet from the ground with a safety monitor, however the safety monitor was performing job tasks and not solely performing the job of safety monitor.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed: 3505 W 13th St., Grand Island, NE 68803 (Job-site) The employer failed to ensure that ladders were utilized within the scope of their intended design. Employees used an 8 aluminum folding step ladder to attach a rope to the structure for fall restraint leaning the ladder against the building wall instead of opening it up as it is intended to be used.
