29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  On or about February 24, 2016, on the jobsite, an employee was exposed to a fall hazard when working from a JLG Aerial Lift basket without a fall protection system.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems.  On or about February 24, 2016, south side, on the jobsite where an employee was exposed to a 20 foot fall when welding washers to the deck without being protected by a personal fall arrest system.    The Sonny Steel Erectors, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.760 (a) (1), which was contained in OSHA inspection number 903019, citation number 1, item number 1 and was affirmed as a final order on 12/23/2013, with respect to a workplace located at 3444 Katy Freeway, Houston, TX.  The Sonny Steel Erectors, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.760 (a) (1),  which was contained in OSHA inspection number 893934, citation number 1, item number 1 and was affirmed as a final order on 12/16/2013, with respect to a workplace located at 19235 Katy Freeway, Katy, Texas.  The Sonny Steel Erectors, Inc. was previously cited for a violation of an equivalent standard of this Occupational Safety and Health Standard 29 CFR 1926.501 (b)(10), which was contained in OSHA inspection number 540959, citation number 1, item number 1 and was affirmed as a final order on 2/4/2013, with respect to a workplace located at 107 W. Rankin Road, Houston, TX 77090.
