MEK & Company, LLC dba Dutch Pantry Family Restaurants; outside of office - On or about 2/25/16, and at times prior thereto, there was a puddle of water outside of the office area due to a leak in the roof which created a slip/fall hazard.      B.)  MEK & Company, LLC dba Dutch Pantry Family Restaurants; near ice machine - On or about 2/25/16, and at times prior thereto, ice was located on the floor near the ice machine which created a slip/fall hazard.    C.)  MEK & Company, LLC dba Dutch Pantry Family Restaurants; near ice machine - On or about 2/25/16, and at times prior thereto, the filter system for the ice machine was dripping water onto the floor and created a slip/fall hazard.

B.)  MEK & Company LLC, dba Dutch Pantry Family Restaurants - On or about 2/25/16, and at times prior thereto,  an extension cord was used to power a  Black and Decker toaster in the kitchen.    C.)  MEK & Company LLC, dba Dutch Pantry Family Restaurants - On or about 2/25/16, and at times prior thereto,  a surge protector was used to power a Farberware toaster in the kitchen.
