Capacity, operation, and maintenance instruction plates, tags or decals of the powered industrial truck were not changed accordingly:      (a) Location - 181 Stedman Street, Lowell, MA:    On or about 2/24/2016, the Toyota Model 7FGCU45 Serial #71088 forklift was operated with a telescoping boom attachment, and the forklift's data plate was not changed to reflect the operating capacity as configured.
