29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    (a) Jobsite:  On or about February 24, 2016, employees were exposed to approximately a seven foot three inch fall hazard when they were using the approximately 14 foot long, 2 foot wide metal gang plank to access the work area.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or 1926.652(c):     (a) Jobsite:  On or about February 24, 2016, two employees were working in an approximately  7 foot deep excavation and they were not protected from a cave in.
