29 CFR 1910.147(c)(4)(i):  Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  The employer is failing to protect employees from caught-in, struck-by, and amputation hazards.  This was most recently documented on February 25, 2016 in the Press Room at Challenge Manufacturing, Inc., located at 10501 NW Transcon Dr. in Kansas City, Missouri.  Inadequacies were discovered in machine specific lockout/tagout procedures for machines such as but not limited to the Eagle Presses models SE4-1500-256-96 (serial number EP362-031515), SE4-1250-240-96(serial number EP-366-0615) described by the company as  press 700 and 701.

29 CFR 1910.147(c)(5)(ii):  Lockout devices and tagout devices were not singularly identified; were not the only devices used for controlling energy; were used for other purposes; and did not meet the requirements of subparagraphs (A) through (D):  The employer is failing to protect employees from caught-in, struck-by, and amputation hazards.  This was most recently documented on February 25, 2016 in the Press Room at Challenge Manufacturing, Inc., located at 10501 NW Transcon Dr. in Kansas City, Missouri.  Authorized maintenance employees are using multiple types of devices for locking out, locks are not standardized, and are not assigned to employees, locks are not identifiable.  Equipment included but was not limited to the AIDA Presses model number NSTS2-8000(1)-425-150 (serial number A7008-0101), described by company as presses 702 and 703.

29 CFR 1910.178(p)(1):  Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been  taken out of service until restored to safe operating condition(s).  The employer is failing to protect employees from struck-by, fire and explosion hazards.  This was most recently documented on February 25, 2016 in the Die Area at Challenge Manufacturing, Inc., located at 10501 NW Transcon Dr. in Kansas City, Missouri.  An employee was operating a Clark forklift Model TC-400L without the rubber cover on the brake pedal and the propane tank was not secured or latched to the fuel tank bracket.

29 CFR 1910.305(g)(2)(iii):  Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:    The employer is failing to protect employees from an electrical  shock hazard.  This was most recently documented on May 17, 2016 in the Press Room at Challenge Manufacturing, Inc., located at 10501 NW Transcon Dr. in Kansas City, Missouri.  Exposed energized conductors were discovered in the flexible cord plugged into the metal outlet box on the AIDA press model number NSTS2-8000(1)-425-150 (serial number A7008-0101).  Strain relief was not provided in that the flexible cord had broken free of the plug end connection.
