Jeffrey Wettberg dba Hectic Housing Renovation Services, 4995 Station Road, Erie, PA  16510, on or about February 26, 2016 and times prior thereto:  A generator was providing electrical power to flexible electrical extension cords.  The flexible electrical extension cords were powering equipment including, but not limited to, the InsulMax Blow-In Insulation Machine.  The flexible electrical extension cords were running under a sliding wooden door.  The cords were not protected against physical damage created by the sliding door.
