29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative  a. At the establishment, employees were exposed to amputation and crushing hazards, in that, there was no lock out tag out program in place to prevent the release of stored energy while repairing machinery, such as but not limited to: the Boarke Sander model # BKM-37 K and S/N FC95380; Router RT#2 Model # NC-1925 PT/TC2 and S/N FAANCPT96054; Hoffman Saw model #MS35SF; Router Model # M30120000; Rockwell radial saw model #14-RAS; and ResawSR900, on or about 02/25/2016.

In the molding area, employees were exposed to an amputation hazard, in that, there was no lower guard on the Rockwell model #14 RAS, S/N FC-1893 used to cut molding, on or about 02/25/2015.
