29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)    At the 107 Meridian St. jobsite: On March 3, 2016, and at times prior thereto, the employer exposed employees to fall hazards of 8 ft. 6 in. to the ground, in that roofing repairs were being completed without fall protection on a 4:12 residential roof.

29 CFR 1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii):  a)    At the 107 Meridian St. jobsite: On March 3, 2016, and at times prior thereto, the employer exposed employees to fall hazards of 8 ft. 6 in. to the ground, in that roofing repairs on a 4:12 residential roof were being completed without receiving fall-protection training by a competent person.
