29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:  WORKSITE: The personal fall arrest systems, used by employees while installing roofing materials on a 6 in 12 pitch roof were rigged such that the employees could free fall more than 6 feet.     JJ Roofing & Remodeling LLC was previously cited for a violation of an equivalent Occupational Safety and Health standard 29 CFR 1926.501(b) (13), which was contained in OSHA Inspection Number 1060296, Citation Number 1, Item Number 1a, issued on 6/01/2015 and affirmed as a final order on 8/10/2015 with respect to a workplace located at 522 Stillman Street, Bridgeport, CT.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  WORKSITE: The employees, exposed to fall hazards approximately 24 feet to the ground while installing roofing materials on a 6 in 12 pitch roof were not provided with information and training in the hazards associated with falls or the procedure necessary to eliminate or minimize those hazards.   JJ Roofing & Remodeling LLC was previously cited for a violation of this Occupational Safety and Health standard 29 CFR 1926.501(b) (13), which was contained in OSHA Inspection Number 1060296, Citation Number 1, Item Number 1b, issued on 6/01/2015 and affirmed as a final order on 8/10/2015 with respect to a workplace located at 522 Stillman Street, Bridgeport, CT.
