This was most recently documented on or about March 1, 2016 at the Under Armour Store jobsite, located at 1843 Village West Parkway in Kansas City, Kansas, where employees were installing a new Heating, Ventilation, and Air Conditioning unit while the 480 volt electrical power was not removed and rendered inoperative. The employer did not initiate and maintain a program which provided for regular inspections of the job site which allowed an employee to perform work in the area of live electrical equipment.  The employee sustained fatal injuries after contacting the energized electrical component.
