29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):  The employer is failing to protect employees from electrocution hazards associated with live parts of equipment or the unexpected energization of electrical equipment  when installing HVAC equipment.  This was most recently documented on or about March 1, 2016 at the Under Armour Store jobsite, located at 1843 Village West Parkway in Kansas City, Kansas, where employees were installing a new Heating, Ventilation, and Air Conditioning unit while the 480 volt electrical power was not removed and rendered inoperative. The employer did not initiate and maintain a program which provided for regular inspections of the job site which allowed an employee to perform work in the area of live electrical equipment.  The employee sustained fatal injuries after contacting the energized electrical component.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:    The employer is failing to protect employees from electrocution hazards associated with live parts of equipment or the unexpected energization of electrical equipment  when installing Heating, Ventilation, and Air Conditioning  equipment.  This was most recently documented on or about March 1, 2016 at the Under Armour Store jobsite, located at 1843 Village West Parkway in Kansas City, Kansas, the employees were not instructed in the recognition of the hazards associated with working on or around live electrical circuits or equipment and the methods to mitigate those hazards.
