29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  a) At 1264 West Beach Boulevard Gulf Shores, Alabama:  On or about March 1, 2016 and at times prior; the employer failed to ensure employees working from an elevated aerial lift basket was wearing a harness and tied into the basket exposing them to a fall hazard of up to twenty feet to the ground below.
