29 CFR 1910.119(d)(3)(ii): The employer shall document that equipment complies with recognized and generally accepted good engineering practices. On or about March 8, 2016, the employer did not document that its equipment complies with recognized and generally accepted good engineering practice according to the ASHRAE-15 and IIAR-2 ANSI Standards or equivalent. Example include, but are not limited to: a. East engine room - the "West", "South" and "East" doors were not tight fitting, permitting passage of escaping ammonia to other work areas (maintenance shop). b. East engine room, "West", "East", and "South" doors - Emergency shutdown procedures including precautions to be observed in case of breakdown, or leak, were not posted outside of each engine room, immediately adjacent to each door. c. East engine room - Inside the "East" engine room there was no visual alarm. Also the meaning of each alarm shall be marked by signs near the annunciators. d. East engine room - Outside the "South" and "East" engine room doors there were no visual and audible ammonia alarms. Also the meaning of each alarm shall be marked by signs near the annunciators. e. East engine room - Outside the "South" and "East" engine room door a remote control for the mechanical equipment in the refrigeration machinery room was not provided immediately outside the machinery room door solely for the purpose of shutting down the equipment in an emergency. f. East engine room - Outside the "South" and "East (overhead door)" engine room there was no legible and securely attached sign to each entrance reading "Engine Room - Authorized Personnel Only". g. East engine room - piping passing through the interior walls of the "South" wall was not tightly sealed to the walls through which it passes, permitting the escape of ammonia to other work areas. h. West engine room - Emergency shutdown procedures including precautions to be observed in case of breakdown, or leak, were not posted outside of the engine room's "West" door, immediately adjacent to the door. I. West engine room - the "West" and "East" blast doors were not tight fitting, permitting passage of escaping ammonia to other work areas.
