Materials or equipment were placed, either permanently or temporarily, within the exit route. a) On or about March 8, 2016, in building 130-55 180th Street, Jamaica, NY 11434 Exit route at southwest corner of building was obstructed by stored materials next to and leading to the exit door, including metal poles and fittings, boxes and bags. b) On or about March 8, 2016, in building 130-45 180th Street, Jamaica, NY 11434 Exit route at west side of building was obstructed by stored materials and equipment next to and leading to the exit door, including a forklift, a forklift charging station, a portable eye wash station and pallets waiting for shipment. c) On or about March 8, 2016, in building 130-54 180th Street, Jamaica, NY 11434 Exit route at south side of building was obstructed by stored materials inside the entry room, including stored boxes, containers and a portable pedestal fan. d) On or about March 8, 2016, in building 130-54 180th Street, Jamaica, NY 11434 Exit route at east side of building was obstructed by stored boxes, a couch and a weights bench located in front of exit door. e) On or about March 8, 2016, in building 130-40 180th Street, Jamaica, NY 11434 Exit route at north side of building adjacent to bay door was obstructed by stored boxes of product and a piece of plywood located in front of the door. f) On or about March 8, 2016, in building 130-45 180th Street, Jamaica, NY 11434 Exit route at west side of building in the area where the 55 gallon drums of chemicals are stored was partially obstructed by the hose of an air compressor strewn on the floor in front of the exit door. 29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition. a) On or about July 12, 2016, in building 130-45 180th Street, Jamaica, NY 11434 Hoses, cardboard, wood and lengths of pipe used to extend piping system into mixing tanks were strewn around the floor of the mezzanine levels above the mixing area, presenting a trip and fall hazard. b) On or about July 12, 2016, in building 130-45 180th Street, Jamaica, NY 11434 The floor of the north mezzanine level above the mixing area and the floor around the power mixing tanks on the ground level was covered in white powder from chemicals such as solid Sodium Hydroxide and Soda Ash. c) On or about July 12, 2016, in building 130-45 180th Street, Jamaica, NY 11434 3M Safety-Walk tape, which had been laid on steps on both sets of stairs leading to the mezzanine above the mixing area, was deteriorating and peeling off, presenting a trip and fall hazard if someone's feet became entangled. The upper pallet was leaning in an unstable position due to its support being undermined by the removal of boxes from the top layer of the lower pallet.
