29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On or about February 29, 2015, at the above addressed jobsite, two employees performing trim work on a commercial building without the use of fall protection were exposed to a 15-foot fall hazard.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet:  On or about 22 October, 2015, at the above addressed jobsite, two employees were utilizing an aluminum extension ladder to access the roof without extending the ladder at least 3 feet above the upper landing surface, exposing the employees to a fall hazard of approximately 12 feet.
