29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes by covers:  a) On or about 3/9/16, at the basement area of the site, Buffalo, NY. The walking/working area had holes from metal studs, that were 12 inches on centers, on the floor. The holes of 12x12 inches were not protected by covers. Employees were exposed tripping hazard.   NO ABATEMENT CERTIFICATION REQUIRED
