29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.  On or about March 7, 2016, employees performing siding installation operations were exposed to struck by hazards while employees handed equipment and material to the second level from the first level.  The employer failed to mandate the use of hard hats.    Abatement certification is required of this item in accordance with requirements of 29 CFR 1903.19(c).

On or about March 7, 2016, employees performing siding installation were exposed to drowning hazards while working over or near water when the employer failed to provide U.S. Coast Guard-approved life jacket or buoyant work vests.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On or about March 7, 2016, employees performing siding installation were exposed to fall hazards greater than 6 feet and the employer did not ensure employees were utilizing fall protection.  Abatement certification is required in accordance with the requirements of 29 CFR 1903.19(c).
