29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:  (a) Roof, new residential construction located at 1370 Brandywine Trail, Alpharetta, GA 30009: Employee was exposed to falls of approximately 10-feet, 3-inches to the lower level while working from a 4/12 pitched roof which was tied into a 10/12 pitched roof with no fall protection system in place. The employee was cutting plywood and running it up the roof to be installed and was exposed to falls as a personal fall arrest system was not worn.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a) Roof, new residential construction located at 1370 Brandywine Trail, Alpharetta, GA 30009: The employer failed to train all employees and provide potentially exposed employees with the procedures necessary to work at heights with a proper fall arrest system.
