29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) Worksite: An employee was doing framing work on an approximately 3-12 pitch roof approximately 15 ft. above the ground without fall protection; on or about 3/5/16.  b) Worksite: Employees were doing framing work up to and including an approximately 5-12 pitch roof and approximately 25 ft. above the ground without fall protection; on or about 3/11/16.    Note:  In addition to abatement certification the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) Worksite: An employee was doing framing work approximately 15 ft. above the ground on an approximately 3-12 pitch roof and had not been trained in recognizing and minimizing fall hazards; on or about 3/5/16.  b) Worksite: Employees were doing framing work up to and including approximately 25 ft. above the ground on approximately 3-12 to 5-12 pitch roofs and had not been trained in recognizing and minimizing fall hazards; on or about 3/11/16.   Note:  In addition to abatement certification the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided:  a) Worksite, front of house: Employees were accessing the interior of the new residential construction to do work and transport materials. The employees used a block of wood to access the landing approximately 32 in. above the ground and were not provided with a stairway or its equivalent; on or about 3/11/16.     Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
