29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system: a) On or about 3/14/16 at a 2 story residential project located at 54 Camelot Drive, West Seneca, New York; employees replacing siding were using a ladder jack scaffold to perform work. The platform height was approximately 10 feet and there was no personal fall arrest system in place. NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: a) On or about 3/14/16 at a 2 story residential project located at 54 Camelot Drive, West Seneca, New York; employees replacing siding were working from the roof. The eave height was approximately 10 feet and there were no fall protection measures in place. NO ABATEMENT CERTIFICATION REQUIRED
