29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):    During the roofing job site at 300 Country Trail Dr., Lake St. Louis, on March 16, 2016, while the project manager for Delta Exteriors was in the yard of the house being worked on, where employees were not using fall protection harnesses, or any other fall protection while installing shingles on the two story house (approximately 19 ft eave to ground and 6 to 12 and 12 to 12 roof pitch), and where at least one worker used a life line nailed to the roof without an anchor, the project manager was not provided competent person training in order to conduct inspections to control and or eliminate hazards.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  During the roofing job site at 300 Country Trail Dr., Lake St. Louis, on March 16, 2016, while the project manager for Delta Exteriors was in the yard of the house being worked on, where employees were not using fall protection harnesses, or any other fall protection while installing shingles on the two story house (approximately 19 ft eave to ground and 6 to 12 and 12 to 12 roof pitch), and where at least one worker used a life line nailed to the roof without an anchor,  the project manager was not provided competent person training, training on fall hazards, and other training for controlling or eliminating construction hazards, such as overhead hazards, as well as for enforcing construction standard requirements.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    During the roofing job site at 300 Country Trail Dr., Lake St. Louis, on March 16, 2016, employees did not use fall protection harnesses, or any other fall protection while installing shingles on a two story house (approximately 19 ft eave to ground and 6 to 12 and 12 to 12 roof pitch).  The project manager was on site at the time of the work and fall hazards.
